Steps to Request a Report:
1. Check Report Availability:
- Send an email to the AFA Manager at actfr.alarms@act.gov.au to request the incident report with information to help identify the incident. For example.
- ACTF&R Incident Number (if known)
- Incident Address
- Incident Date
- Incident
- Time
- You will receive a confirmation email within 14 working days. This email will include an Incident Reference Number and an “Authority to Release Information” form that you need to complete.
2. Confirm and Accept Charges:
- If you are willing to proceed and accept the associated charge for receiving the incident report, please reply to actfr.alarms@act.gov.au detailing the type of report you wish to request and attach:
- The completed “Authority to Release Information” Form.
- All associated documents listed on the “Authority to Release Information” Form.
- Invoice Details: Provide the necessary details for creating an invoice, including; Name, Address, Date of birth (for individuals), ABN (for businesses), email, and phone number.
3. Receive the Report:
- If your form is accepted, you will receive the redacted incident report and any associated pictures via email.
- An invoice will be sent to you from the ACT Gov Shared Services Finance department after the report is emailed.
4. Pay the Invoice:
- Pay the invoice via Electronic Funds Transfer (EFT) within 30 days.

Types of Reports Available:
Incident Report - $55.00 (Item 273):
Description: A written summary of any incident attended by ACT Fire & Rescue. This includes rescues, motor vehicle collisions, and hazardous materials incidents.
Fire Incidents: For fire incidents, the Officer in Charge (OIC) will determine the cause and origin based on their training, experience, and evidence gathered at the scene.
Fire Investigation Report - $572.00 (Item 274): (not conducted for every incident and therefore often not available)
Description: A comprehensive document compiled by specialist ACT Fire & Rescue Fire Investigation officers. This report aims to determine the cause and origin of a fire.
Investigation Basis: The report is based on witness reports, scientific observations, and evidence collected. A formal Fire Investigation, often decided in conjunction with ACT Police, is required for this report.
